Technical Assistance:
Discussed the process and requirements for changing the center's policy on administering medications.

Comments:
An unannounced monitoring inspection was completed today. The physical plant, 4 staff files, 5 children's files,staff training, children's activities, emergency procedures, drills and supplies were inspected between 12:00pm-1:30pm. There were 19 children under the supervision of 4 staff. During the inspection, children were observed playing outside on the playground, eating lunch, reading books, learning about geography and working with crafts. Each day the children are in care, they bring their lunches from home. There is now 1 staff certified to administer medications.The interactions between the children and staff were seen to be positive and constructive in nature. Standard #: 22VAC40-185-130-C
Description: Based on file review, 2 out of 5 children's records do not have additional immunizations once between each child's fourth and sixth birthdays.
Evidence
1. Child #3 is 8 years old and child #4 is 6 years and 5 months old. Neither of these children have this documentation on file.

Plan of Correction: We will ask the parents to bring this into the center.

Standard #: 22VAC40-185-60-A-7
Description: Based on file review, 1 out of 5 children's files does not have all required information.
Evidence
1. Child #5 has documentation of having an intolerance to tree nuts. However, there is no documentation of an action to take in an emergency situation

Plan of Correction: We will ask the parents to add this information to the file.

Standard #: 22VAC40-185-280-B
Description: Based on observations, there were hazardous substances which were not kept in a locked place, inaccessible to the children in care.
Evidence
1. In the corner of the main classroom, where the children keep their belongings, there was a bottle of cleaner and a can of spray paint on a cabinet.

Plan of Correction: These items will be moved.

Standard #: 22VAC40-185-330-B
Description: Based on observations, there were areas under and within the fall zones areas of playground equipment that did not have meet the resilient surfacing guidelines.
Evidence
1. Under the blue and red climbing apparatus and the silver slide, there were areas that did not have a minimum of 6 inches of mulch.

Plan of Correction: We will get more mulch.

Standard #: 63.2(17)-1721.1-B-3
Description: Based on file review, 1 out of 4 staff record does not have results of a search of the central registry maintained by any other state in which the individual has resided in the preceding five years.
Evidence
1. Staff #3 has lived in Maryland within the past five years, but does not have a Search of the Central Registry for that state on file.

Plan of Correction: This will be sent out within the next week. A 30 day follow-up will be done if needed.
