Technical Assistance:
Discussion was held on staff records and safe sleep.

Comments:
An unannounced monitoring inspection was conducted today from approximately 11:40 am 3:45 pm. There were 111 children supervised by17 direct care staff. The physical plant, 10 staff records, 10 children?s records, children's nutrition, evacuation drills, injury reports, emergency supplies were inspected. Medications were on-site today and records were reviewed. The children were observed transition to napping. Hand washing was observed. There were age appropriate materials for all children.

Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on staff records review, 3 out 10 records, the center did not maintain documentation at least every two years from the date of the first initial screening or testing, staff members obtained and submit the results of a follow-up tuberculosis screening.
Evidence: Staff # 8 (last dated screening 8/27/2017) and Staff #9 (last date screening 6/27/201) did not have a current results form tuberculosis screening.

Plan of Correction: I gave the form for TB and staff will get test this weekend.

Standard #: 22VAC40-185-70-A
Description: Based on review of staff records, 1 out 5 records, there was no documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering.
Evidence: Staff # 4, hired 2/18/2020, does not have documentation of two or more references.

Plan of Correction: I will call and get reference checks.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and furnishings of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
1. In the classroom of toddlers, on yellow cube and blue cube and three rolling balls had torn material that was accessible to children.

Plan of Correction: The toys were removed from area that was accessible to children.

Standard #: 63.2(17)-1720.1-A
Description: Based on review of three staff records it was determined that not all staff repeated background checks every five years.
Evidence:
1. The most recent Criminal Registry check for Staff #6 was dated 11/20/2014.
2. The most recent Central Registry check for Staff #9 was dated 10/25/2014.
3. The most recent Central Registry check for Staff #10 was date 10/23/2014.

Plan of Correction: Staff were given from to complete today and will mail off today.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review, center did not obtain the results of a out of state central registry for employees.
Evidence: 1. Staff # 3's records indicated that she has resided outside of Virginia within the last 5 years. An request for an out of state central registry was not sent out for Staff # 3. Staff # 1 was hired on 12/19/2019

Plan of Correction: Staff completed form and will mail out.
