Comments:
An unannounced renewal inspection was conducted today from 9:00am to 11:30am with the Owner/Director. There were three children in care, ranging from three years to five years of age, supervised by one staff. The children were observed having morning snack and washing their hands for circle time. Five children?s records and six staff records were reviewed and were in compliance. The Center has six staff with current certification in CPR and First Aid, and five staff trained in Daily Health Observation Training. The center does not administer medication. The attendance and emergency drill log were reviewed and were in compliance. The most recent evacuation drill was documented 2/10/20 and the most recent shelter-in-place was documented 7/5/19. The first aid kit, flashlight, and battery operated radio were observed and were in compliance. The most recent Fire Inspection on file was dated 3/26/19 and another is scheduled for later this month and the most recent Health Inspection on file was dated 11/14/19. Morning snack today was pretzel sticks, grapes, and water. Description: Based on observation and interview with the Director, not all areas and equipment of the center, inside and outside, were maintained in a safe and operable condition. Evidence: 1. In the Preschool classroom one of the toilets was covered with tape and paper marking it out of order and the Director stated that it had been out of order for a few months. 2. On the playground there was a plastic castle play structure that hard a large crack about six inches long on one side and a hole on the other side of approximately three inches in diameter that both posed a scratching hazard.

Plan of Correction: We will remove the castle today. We have had one person come out to look at the toilet who could not fix it and we are scheduling with another.

Description: Based on observation and interview with staff, children three years of age were offered food that is considered to be a potential choking hazard. Evidence: Child A, with a birthday of 5/20/16, was observed eating whole grapes as part of morning snack.

Plan of Correction: Moving forward we will make sure to cut the grapes for children under 4.
