An unannounced complaint inspection was initiated on 02/19/2020 in response to a complaint regarding supervision and injury reporting. The Department was notified on 02/03/2020. The inspectors interviewed staff, made observations at the center, and reviewed documentation at the facility on 02/19/2020 from 9:05 to 11:05 AM. Additional interviews were conducted on 02/19/2020, 02/24/2020 and 02/26/2020. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid.
