Comments:

An unannounced monitoring inspection was conducted from 11:40 am-2:30 pm. At the time of entrance, 12 children were in care with 3 staff members present. Children were observed playing outside, handwashing, and preparing for lunch. The children were served meatballs, polenta, peas, fruit, and milk. A selection of 5 staff and 5 children records, the physical space, evacuation drills, attendance records, and injury reports were reviewed. There were no medications on site today. Standard #:	22VAC40-185-160-A
Description:	Based on record review, the center did not obtain documentation of a negative tuberculosis screening no later than 21 days after employment for one staff member. Evidence: Staff #3 began employment on 6/6/19. Documentation of the tuberculosis screening for Staff #3 was dated 7/17/2019.
Plan of Correction:	Documentation of tuberculosis screening was obtained and placed in the file. When we have new hires I will set a reminder and put information in multiple calendars to make sure we are completing within the time frame.
Standard #:	22VAC40-185-60-A
Description:	Based on record review, 1 of 5 children's records did not contain all required information Evidence: The record for Child #1 did not contain the address of two designated people to call in an emergency if a parent cannot be reached.
Plan of Correction:	Parents were emailed during inspection.
Standard #:	22VAC40-185-70-A
Description:	Based on record review, 2 of 5 staff records did not contain all required information. Evidence: 1. The record for Staff #1 (date of hire 9/2/19) did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering. 2. The record for Staff #2 (date of hire 12/8/19) did not contain documentation of the address of a person to be notified in an emergency.
Plan of Correction:	I will speak with staff regarding missing information and will contact references.
Standard #:	22VAC40-185-550-M
Description:	Based on record review, 2 of 2 injury reports did not contain all required information. Evidence: 1. 1 of 2 injury reports did not contain the date and time parents were notified, and documentation on how parent was notified. 2. 2 of 2 injury reports did not contain any future action to prevent recurrence of the injury
Plan of Correction:	I will talk with the teachers individually and during staff meeting to remind them to fill the forms out completely and answering all questions included on the form.
