Technical Assistance:
Technical assistance today included required postings, maintaining a clear escape path during naptime; managing bathroom changes with a 2 1/2 year old; and sight and sound supervision while children move between classroom and bathroom areas.

Comments:
Complaint # 52848
An unannounced inspection took place today from 11 am - 2:00 pm. There were 30 children present in three classrooms, with five staff present plus the director. A sixth staff member arrived at 12:10 pm. Classrooms were observed, the environment was inspected and staff were interviewed. The complaint was valid for supervision issues (340 A & G) and 560F. The following were not part of the complaint but observed to not be in compliance during the inspection: 280, 440E, 500B3, and the Code of Virginia (conspicuous placement of violation notices). A Violation Notice was given to the director to complete a plan of action for the violations.

Complaint related: No
Description: Based on observation today, the inspection report from the last inspection was not posted in a conspicuous manner as required.
Evidence:
The Violation Notice was taped down with the report facing the wall. On top of it was the Inspection Summary, which was taped on all four sides, leaving the Violation Notice inaccessible.

Plan of Correction: 03/10/2020: It was accidentally stuck together and separated the papers.

Description: Based on observation during today's inspection, hazardous substances were not kept in a locked place as required.
Evidence:
A hallway bathroom had an open door throughout the visit, and the unlocked cabinet under the sink had a bottle of cleaner and deodorizer spray.

Plan of Correction: 03/10/2020: The lysol from the adult bathroom is moved back to the mop room locked cabinet. As an added precaution, we also added child lock in the adult bathroom under the sink cabinet.

Description: Based on observation during today's inspection, staff who were supervising children was not always ensuring their care, protection and guidance.
Evidence:
One child on his way to the bathroom from his classroom at 11:52 am stopped in the open kitchen located in the hallway. He opened a drawer which had mints in it (a choking hazard) and climbed onto the small stepladder and opened the above cabinets. No staff was aware of the child's actions. At 11:55, three children were observed playing without supervision in the hallway. They went into the the bathroom where they opened the "Do not use" bathroom stall and let water fill the sink.

Plan of Correction: 03/10/2020: New staff will be given additional 1-2-1 training in person and in classroom. Child safety gate is added to kitchen as an added precaution.

Description: Based on observation during today's inspection, children were not always within actual sight and sound supervision.
Evidence:
After lunch, one staff in the classroom was sending children to the bathroom which was around a corner and out of sight of the staff.

Plan of Correction: 03/10/2020: Hired a veteran child care specialist to give in person classroom training over several weeks to make sure staff understand sight and sound supervision.

Description: Based on observation today, there were not at least 12 inchees of space between occupied mats at rest time.
Evidence:
In one classroom, there were two areas where mats were side by side, and other areas where the mats were blocking the exit and path to the door

Plan of Correction: 03/10/2020: New staff are trained and mat plan on keeping 1 foot apart.

Description: Based on observation during today's inspection, there were some toileting/diapering areas that did not have all required items.
Evidence:
Although the center does not usually use diapers, today there was an instance where a child needed to be changed. There was no nonabsorbent surface for the child to lie on, and no appropriate disposal for diapers.The child was changed standing up while leaning on the staff member, and he was observed as he fell over during the process.

Plan of Correction: 3/10/2020: Added a diapering mat even though children are supposed to be postty trained for accidents and added extra bins.

Description: Based on observation and staff confirmation during today's lunch, the latest USDA recommendations for the amount of food were not followed.
Evidence:,
The menu for today from the food service directs a portion serving of 1 hamburger and bun per child. Children were given half a hamburger and bun-.but USDA regulations are for 1 1/2 ounces of protein for lunch for children age 3 - 5..

Plan of Correction: 03/10/2020: we coordinated with catering to get the catering numbers correct.sSometimes were have too much and sometimes we have less.
