Technical Assistance:
Discussion on special feeding, hazardous material, topical ointments.

Comments:
An unannounced monitoring inspection was conducted today approximately 12:40 pm to 4:30 pm. There were 94 children supervised by 13 direct care staff. The physical plant, 5 staff records, 10 children?s records, children's nutrition, supplies, evacuation drills were inspected. Medications were on-site today and records were reviewed. The playground and surfacing were inspected. The children were observed in free play, napping, outside play. Hand washing was observed during the inspection. There were age appropriate materials for all children. Staff-to-child ratios were in compliance.

Violations:
Standard #: 22VAC40-185-130-A
Description: Based on children's record review, the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
1. Child #2 (start date 6/24/2019) did not have documentation of immunizations before attending the center available for review.

Plan of Correction: We will get results from parents and update record this week.

Standard #: 22VAC40-185-130-B
Description: Based on children's record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence: Child #8's(date of birth 3/09/2018) and Child #10'[s (date of birth 11/08/2018) record did not have documentation of additional immunizations once every six months.

Plan of Correction: We will get results from the parent and put in record this week.

Standard #: 22VAC40-185-140-A
Description: Based on children's records review, 1 out 10 records ,the center did not have a physical examination by or under the direction of a physician.
Evidence:
1. Child # 2 (start date 2/08/2019) did not have documentation of physical within 30 days available for review.

Plan of Correction: We will get results from the parent and update record this week.

Standard #: 22VAC40-185-160-C
Description: Based on staff records, 1 out 7 records, the center did not maintain documentation that at least every two years from the initial first screening staff members obtain and submit the results of a follow-up tuberculosis screening.
Evidence: Staff #2 (date of employment 10/25/2010), last documented screening in record was 2/13/2017.

Plan of Correction: To receive a copy of TB test on or before March 20, 2020.

Standard #: 22VAC40-185-280-H
Description: Based on observation, the center did not ensure purses or pockets that are accessible to children.
Evidence: In the three-year-old classroom, a purse was on table that was accessible to children.

Plan of Correction: Staff will be retrained.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review, center did not obtain the results of a out of state central registry for employees
Evidence: 1. Staff # 7's records indicated that she has resided outside of Virginia within the last 5 years. An request for an out of state central registry was not sent out for Staff # 7. Staff # 7 was hired on 11/15/2019.

Plan of Correction: It will be submitted today.
