Comments:
An unannounced monitoring inspection was conducted on March 11, 2020 from approximately 10:15 am to 12:45 pm. Eleven staff were present with sixty-four children during the tour of the facility. Additional staff were on site for administrative duties. Five children's and four staff records were reviewed. Children were observed engaged in various activities inside and playing outside.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. In the two's room there was a radio in the window sill leaving a dangling cord within reach of children.
2. In the Pre-2 room there was a pencil sharpener on a tall shelf with a dangling cord within reach of children.

Plan of Correction: Director had teachers correct the issues during the inspection. Staff will be reminded in an upcoming training that all cords are to be secured out of reach of children.
