Discussion was held on the topics of current enrollment, current staffing, and to remove the cover sheet over the posted violation notice.

The following inspection took place between the hours of 3:30 pm through 5 pm. Today, I met with the Staff of the YMCA after-school program located on site at John Tolbert Elementary school located in the town of Leesburg, Virginia. At the start of this unannounced monitoring inspection there were a total of 43 children observed with 3 staff; good ratio for this 1 group of school age children. A total of 5 children?s files were reviewed. Staff files shall be reviewed on a later date and through the YMCA administrative office in Leesburg. The children were engaged in the following: large motor skilled activities on the playground, bathroom break, hand washing, snack, homework time, games, art, and other organized activities. Standard #: 22VAC40-185-60-A Description: Based on review, 2 of 5 children files reviewed indicated that not all of the required information/documentation was contained within each child's record.
Evidence -
1. On the date of inspection (03/04/2020), Health information, as required by 22 VAC40-185-130 (Immunization Page) through 22 VAC40-185-150 (Physical Examination), was not available for review for Child #1, and Child #2 .
2. While the center is located on the same premises where both children attend school, there was no statement verifying the school's possession of each child's health record, nor were these school's records accessible during the center's complete hours of operation.Plan of Correction: The Parent of each applicable child shall be contacted, the documentation obtained, and placed on file.
