Discussion was held on the topics of current enrollment, current staffing, and the current theme for the week - Dr. Seuss.

Comments:
The following inspection took place between the hours of 9:30 am through 12 pm. Today, I met with the Staff of the Douglass Community Center located in the town of Leesburg, Virginia. At the start of this unannounced monitoring inspection there were a total of 29 children observed with 4 classroom staff; good ratios in both rooms. A total of 6 children?s files and were reviewed. Staff files shall be reviewed on a latter date and through the PRCS main office. The children were engaged in the following: center activities in their room, large motor skills on the playground, pajama day (keeping in line with this week's theme, and other organized activities. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review and interview, 1 of 6 children's files indicated that each child did not have a physical examination by or under the direction of a physician within one month after attendance.

Evidence -
1. On the date of inspection (03/06/2020), documentation of a physical examination was not available for Child #6.
2. Center Staff stated (during an interview conducted on the date of inspection) that the parent of Child #6 had obtained this documentation, but has yet to bring it in to be included in the child's record. Staff also stated that the start date for Child #6 was 10/02/2019.

Plan of Correction: The parent shall be contacted and this documentation shall be obtained.
