Technical Assistance:
Discussion on medications, program qualifications, staff records.

Comments:
An unannounced monitoring inspection was conducted today from approximately 9:42 am to 12:45 pm. There were 82 children supervised by13 direct care staff. The physical plant, 6 staff records, 8 children?s records, children's nutrition, evacuation drills, injury reports, emergency supplies were inspected. Medications were on-site today and records were reviewed. The children were observed in free play, center activities, napping, outdoor play, and transitioning. There were age appropriate materials for all children.

Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on children records review, 1 out 6 records, the center did not maintain documentation of additional immunizations once every six months for children under the age of two years.
Evidence: Child #3, date of enrollment 12/31/2018, last documented immunization available in record was 3/06/2019.

Plan of Correction: Parents have been notified of missing documentation. Contacted parent again during licensing them that we have still not yet received physical. Parent will have physical to use by close of business Friday, March 13 2020 or they will not return until physical is in file.

Standard #: 22VAC40-185-140-A
Description: Based on a review of children records, each did does not have within one month after attendance a completed physical examination.
Evidence: Child # 2, dated of enrollment 1/21/2019, did not have a completed physical examination available for review during the inspection.

Plan of Correction: Parents have been notified of missing documentation. Contacted parent again during licensing visit to remind them that we needed updated immunizations. Child has 18-month check-up scheduled for March 10, 2020. Parents will bring in updated vaccines immediately after the appointment.

Standard #: 22VAC40-185-160-C
Description: Based on staff records review, 1 out 6 records, the center did not maintain documentation that staff members at least every two year from the date of the first initial screening or testing, obtain and submit the results of a follow-up tuberculosis screening.
Evidence: Staff #1, date of employment 8/14/2012, last documented tuberculosis screening results were 1/08/2012.

Plan of Correction: Staff had been reminded that TB test was expiring and to provide updated documentation. At the time of the licensing inspection when it was discovered the update had not been done, the staff member immediately left to have a chest x-ray. Due to immunizations received in the past, this employee test positive and must have a chest x-ray. Staff shows no sign of tuberculosis.
