22VAC40-185 STAFFING AND SUPERVISION.

Comments:
A self reported incident involving care and protection and supervision of one child was investigated jointly with another local government agency on 2/6/20. Child and staff interviews were conducted on site on 2/6/2020 and one staff interview conducted on 2/10/20 off site. Incident report and the center's own policies and procedures were reviewed. Observation of the hallway leading from the cafeteria to the exterior doors leading to the outdoor playground was made. During the investigation, violations were determined pertaining to care and protection, supervision and policies and procedures. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on staff interviews and review of the staff handbook, the licensee failed to maintain compliance with the center's own policies and procedures.
Evidence:
1. On 2/4/2020, Staff failed to follow the Center's own policy "5.2.1 Daily Attendance/Absence". This policy requires that "All children must be accounted for at the beginning of the program using the Supervisor sign in/out lo"

2. Staff failed to follow the center's own policy "4.1.1 Site Sign In/Out Book". This policy states "Staff is responsible for taking attendance at the start of the program each program day. The book must be checked at arrival and staff must be sure that every child is accounted for, each name should either be signed in or the absence is confirmed with the office or parents."
3. On 2/4/2020, Child #1(age 8 years), arrived late to the program at approximately 3pm. Child #1 had stayed after school with their teacher and arrived to the program approximately 10 minutes late. Staff failed to verify that Child #1 had stayed after school with their teacher. The attendance record dated 2/4/2020 instead showed that Child #1 was in attendance at the beginning of the program.

Plan of Correction: All staff were immediately reminded of CASA sign in and sign out procedures. Children arriving late due to other activitieswill be signed in with the time of arrival noted.

Standard #: 22VAC40-185-340-A
Description: Based on staff and child interviews, one staff failed to ensure the care, protection and guidance of a child.
Evidence:
l. On 02/04/2020 at approximately 3pm, Child #1(age 8 years) arrived late to the after school program located in the cafeteria. Child #1 did not see their group in the cafeteria and asked Staff #1 where they were. Staff #1 told Child #1 that the group was outside on the playground. Staff #1 guided Child #1 to the exterior door leading out to the playground.
2. According to Staff #1, they attempted to reach the staff in Child #1's group with a portable radio to inform staff that Child #1 was joining their group outside. Staff #1 stated "I wasn't able to confirm where they were because the walkie talkie wasn't working."
3. According to Staff #1, at the same time they were at the exterior door with Child #1, five children from the CASA program were running in the hallway behind them. Staff #1 stated "I turned around to deal with them and got distracted and turned around and [Child #1] was gone."
4. During this time, Child #1 went outside unsupervised to the playground.Staff #1 did not verify that Child #1's group was on the playground. The playground is located up a hill and approximately 50 feet from the exterior doors leading to the playground. A portion of the area leading to the playground from the exterior doors is obstructed from view due to brick wall that is approximately 4 feet tall.
5 According to Child #1, they realized that their group was not outside on the playground and stated "I ran back to the door and [Staff #1] had left and I was locked out. I tried to knock as hard as I could, then I started to cry".
6. According to Child #1 a parent who was on the playground realized what happened and began knocking on the door. Child #1 stated "a boy who was standing in the hallway finally opened the door".
7.Child #1 entered the building and found Staff #2 standing in the doorway of the gym supervising children entering and exiting the bathroom. Child #1 rejoined their group in the gymnasium which was adjacent to the cafeteria. According to Staff #1, Child #1's group normally is outside on the playground and was unaware that the group was in the cafeteria.
8. Child #1 was locked outside of the building for an undetermined length of time.

Plan of Correction: Staff #1 is no longer employed with CASA. Sight and sound supervision procedures have been reviewed again with all staff.

Standard #: 22VAC40-185-340-F
Description: Based on staff and child interviews, one child was not within actual sight and sound supervision of staff.
Evidence:
l. On 02/04/2020, Child #1 was found outside unsupervised at approximately 3:10pm by a parent who was on the playground. During this time, 5 staff remained in either the cafeteria or the gym.

Plan of Correction: Staff #1 is no longer employed with CASA. Sight and sound supervision procedures have been reviewed again with all staff.
