All staff must complete Virginia Mandated Reporter Recognizing Child Abuse and Neglect Training

Comments:
An unannounced monitoring inspection was conducted today between 9:30 am and 1:30 pm. There were 94 children with 16 staff members. The children were observed making T-Rex foot prints and practicing yoga. They were also observed during hand washing, diaper changing and eating snack. The physical plant, children's records, staff records, evacuation records attendance logs, emergency plans, lunch boxes, medications and authorizations and emergency supplies were inspected. Standard #: 22VAC40-185-60-A
Description: Based on a review of children's records, the center failed to obtain and maintain up to date records for all children enrolled,

Evidence:
Child #3's record did not have documentation of either emergency contacts and did not have authorization for persons other than parents authorized to pick up.

Plan of Correction: I will request the information from parents.

Standard #: 22VAC40-185-240-A
Description: Based on a review of staff records, the center failed to ensure that staff records contain documentation of completed training related to Recognizing Child Abuse and Neglect and the legal requirements for reporting suspected child abuse as required by ???? 63.2-1509 of the Code of Virginia.

Evidence:
Staff #1, #2, #3, #4 and #5 did not have documentation of the completion of the Recognizing Child Abuse and Neglect Training

Plan of Correction: Director is confident that training has been completed, but documentation could not be located during inspection. Documentation will be submitted to LI as soon as it is located.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the center failed to ensure that outlets have protective covers that are of a size that cannot be swallowed by children.

Evidence:
In the large hall, there were three outlets without protective covers.

Plan of Correction: These were replaced during the inspection.

Standard #: 22VAC40-185-500-A
Description: (A) Based on observation, the center failed to ensure that children's hands shall be washed with soap and running water after toileting.
(B) Based on observation, the center failed to ensure that staff shall wash their hands with soap and running water before and after helping a child use the toilet or diaper change.


Evidence:
Children's hands were not washed with soap and running water after diaper changes.

Plan of Correction: (A) I will remind staff that children's hands must be washed with soap and running water after diaper changes.
(B).I will remind staff that they must wash their hands with soap and running water before and after helping a child use the toilet or a diaper change.
