Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on March 10, 2020 from 10:50 a.m. through 1:05 p.m. There were thirty-nine children in the care of nine staff in five classrooms. No medication is being administered at the present. Six staff and five children's records were reviewed and updated. Areas of non-compliance may be found on the violation notice.
Areas observed were: outside play, educational centers, hand washing/bathroom and diapering procedures, infant feeding, lunch and nap. Interactions between the children and staff were observed as appropriate and the children seemed comfortable with their daily routine and schedule.

Standard #: 22VAC40-185-160-A Description: Based on record review, the center failed to have one staff obtain a tuberculosis screening within 21 days of employment.
Evidience:
1. Six staff records were reviewed.
2. Staff #4 did not have documentation of obtaining a tuberculosis screening within 21 days of employment.
3. Staff #4's documented date of hire was 02/03/2020.
4. The director confirmed Staff #4 had not obtained a tuberculosis screening.Plan of Correction: Staff #4 will immediately obtain a tuberculosis screening. The director will send a copy to the licensing inspector once obtained.
Standard #: 22VAC40-185-500-B Description: Based on observation, the center failed to have a non-absorbent surface for diapering in one classroom.
Evidence:
1. Diapering was observed in Classroom #1.
2. The diaper changing pad had a small tear in the center.
3. Staff confirmed the diaper changing pad was torn.Plan of Correction: A new diaper pad will immediately be purchased.
