Comments:
An unannounced monitoring inspection was completed on 3/13/2020. There were 35 children present in four groupings with nine staff directly supervising and one administrative staff present. Five children's records and five staff records were reviewed. One emergency medication was reviewed.

The inspector arrived at the center at 9:30 am and departed at 1:00 pm.

The children were observed during free play times in the classrooms, during music and movement activities, during lunch, and during nap.

There was detailed discussion with the administrative staff person about the renewal process. The inspector also recommended to make contact with the fire inspector to consult on some ways to not restrict exits while keeping children safely in classrooms.

Violations:
Standard #: 22VAC40-185-80-A
Description: Based on observation of records and interview, the center failed to ensure that there was a written record of children in attendance.

Evidence:
1. A classroom was observed to have 12 children present. There were 10 children marked present on the attendance that was observed at 12:00 pm. The two children identified to not be marked present arrived, according to staff interviews, at approximately 8:25 am and 9:00 am. One child is a school-age child who would not normally be in attendance during the day.

2. When staff in the class was made aware of the children not being marked present, the staff stated that the parents did not sign them in.

Plan of Correction: Staff will be reminded to check attendance and ensure all children are marked present.

Standard #: 22VAC40-185-270-A
Description: Based on observation and interview, the center failed to ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe, and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence:
1.There was a wooden loft structure observed in a classroom. One leg of the structure that was observed to be passed by the children several times while playing and was in disrepair creating an unsafe condition. The leg of the wooden structure at the base on floor level up the wooden leg approximately three inches was splintering and jagged. There was also a protruding nail approximately five inches up the wooden leg. The nail was noticed and the inspector felt that the nail was sharp and an unsafe condition for the children. The staff person stated that there was an attachment that was broken off approximately two weeks ago and had been reported to administration to have it fixed.

Plan of Correction: The inspector immediately called this to the attention of the lead staff person in the classroom. The staff person hammered the nail in and covered the splintering area.

Standard #: 22VAC40-185-280-G
Description: Based on observation and interview, the center failed to ensure that hazardous substances not kept in original containers were clearly labeled to indicate their contents.

Evidence:
1. A clear, generic spray bottle was observed in a locked closet in a classroom. The bottle contained a clear, bubbly liquid. There was no label on the bottle that could be seen. The staff was asked of the contents and stated that it was soapy water.

Plan of Correction: This was corrected during the inspection. The spray bottle was labeled.
