Technical Assistance:
Discussed locking closets, the playground area is being worked on, diapering surface, cribs labeling and back classroom are not being used.

Comments:
An unannounced monitoring inspection was conducted on March, 11, 2020 from approximately 10:30 am to 2:30 pm. Five staff were present with 31 children at the onset of inspection. Seven children's records and five staff records were reviewed. Children were observed watching dance activity on TV, exercising, tummy time and freely playing.

Standard #: 22VAC40-185-130-A
Description: Based on record review , the licensee did not ensure each child had immunization documentation before the child can attend the center.

Evidence:
Child #4 did not have immunization documentation in the record.

Plan of Correction: Program director will verify all student immunization records are received and filed for each child prior to their first date of attendance.

Description: Based on record review, the licensee did not ensure staff resubmit TB test results every two years.

Evidence:
Staff #1 TB test most recent results were 1/2/2018. The results were more than two years old.

Plan of Correction: Program director will review all staff records on a monthly basis to ensure updated information is received and filed within a timely manner.

Description: Based on record review, the licensee did not ensure all required documentation was contained in the record for each child.

Evidence:
Child #4 did not have start date, parents employers phone number and one emergency contact complete address with city state and zip code.

Plan of Correction: Program director will review all student files upon enrollment to ensure all required documentation is contained within the record for each child.

Description: Based on observation, the licensee did not ensure hazardous substances are kept in a locked place using a sfe locking method.

Evidence:
In the preschool classroom, under the sink in the bathroom, the cabinet safe lock was broken and there were two cans of shaving cream labeled "Warning and Keep out of the reach of children. "

Plan of Correction: Program director will conduct training with staff members to review expectations for properly storing potentially hazardous art materials.

Description: Based on observation and staff interview, the licensee did not ensure there was one staff member for every 10 children for children from three to the age of eligibility to attend public school.

Evidence:
In the preschool classroom , at 11:05 am there was one staff with 13 children. Staff #1 confirmed there was 13 children and the youngest was three years of age.

Plan of Correction: Program director will ensure additional staff is available to serve as a substitute in the event of unexpected absences to maintain appropriate staff-child ratios.

Standard #: 22VAC40-185-440-B
Description: Based on observation and staff interview, the licensee did not ensure the cribs were identified for use by specific child.

Evidence: In the toddler room, a child was sleeping in a crib that was not labeled. Staff #4 confirmed the crib was not labeled.

Plan of Correction: Program director will require staff to perform daily checks to ensure all labels remain attached to cribs that identify the child designated for use.
