A Subsidy Health and Safety inspection was conducted on March 11, 2020 to review supplemental health and safety requirements. Five staff were present with 31 children at the onset of inspection. Seven children's records and five staff records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.

Standard #: 22VAC40-665-540-A Description: Based on record review, the vendor did not ensure the documentation of the TB test screening was completed with in the last 30 calendar days of the date of employment.
Evidence: Staff #4 date of employment was 12/22/2019 and TB results were 11/04/2019. The TB test was completed more than 30 days before date of employment.Plan of Correction: All new employees will have TB test completed no more than 30 days prior from date of employment.
