Discussed with the administration medication authorization forms.

Comments:
An unannounced monitoring inspection was conducted today from 10:15am-2:30pm. There were 138 children (infants-5yrs.) directly supervised by 33 staff. The physical plant, 10 staff records, 10 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, playing outside, circle time, coloring, practicing letter sounds, discussing values, naptime and eating lunch. Today's lunch consisted of Baked Cheese Ziti, green beans, fresh fruit and milk. Diapering and hand-washing procedures were also observed. There was an adequate supply of materials for the children. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, equipment of the center was not maintained in a safe and operable condition.

Evidence:
In Room 33, a four year old classroom, two chairs in the library area were observed with rips that exposed the interior foam.

Plan of Correction: The chairs were immediately removed from the classroom. The chairs will be covered.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. In Room 38, a four year old classroom, six packages of hand sanitizing wipes were observed inside of the bathroom cabinet. The wipes were stored in an upper cabinet;however, the cabinet was unlocked.
2. In Room 39, a two year old classroom, four cans of air freshner and two boxes of hand sanitizing wipes were observed inside of a bathroom cabinet. The items were stored in an upper cabinet; however, the cabinet was unlocked.
3. In Room 39, a container of bleach and water, disinfectant wipes, multi-purpose cleaner were observed in an unlocked cabinet above the sink.

Plan of Correction: The hazardous substances were removed from the cabinets immediately.

Standard #: 22VAC40-185-280-D
Description: Based on observation, cleaning and sanitizing materials were not stored in areas physically separate from food.

Evidence:
Cleaning supplies(multi-purpose cleaner and bleach and water) were stored in a cabinet with crackers and veggie straws.

Plan of Correction: The cleaning materials and sanitizing materials were removed immediately and the food from the cabinet was removed to another cabinet.

Standard #: 22VAC40-185-510-N
Description: Based on medication review, a parent was not notified that a medication authorization expired. The medication was not picked up within 14 days and the parent did not renew the authorization.

Evidence:
1. One of six medications reviewed had an expired authorization form with medication still on site.
2. The authorization form for Child#1 was completed on 12/9/2019, it needed to be renewed after 10 work days.
3. The form was not signed by the physician to be considered a long term authorization.

Plan of Correction: We will have the parent sign the form today.
