Comments:
An unannounced monitoring inspection was conducted from 10:10 am-12:50 pm. At the time of entrance, 34 children were in care with 12 staff members present. Children were observed during bottle feeding, painting, and sensory play. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, attendance records, injury reports, medications and the Emergency Preparedness Plan were reviewed. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, one of five child records reviewed was incomplete.
Evidence: Child #3 did not have proof of identity in the record.

Plan of Correction: Parent provided updated proof of birth during pick up following the inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation, equipment of the center was not maintained in a safe and operable condition.
Evidence:
A sofa located in the Junior Pre-school (2-3 year old) classroom was ripped and torn to expose filling on the seat and both arms.

Plan of Correction: The sofa was removed from the classroom during the inspection.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the licensed agency did not have a central registry finding within 30 days of employment.
Evidence:
1. Staff #1 and Staff #5 did not have documentation of Central Registry findings.

Plan of Correction: Fingerprint clearances were received prior to hire for all employees. Central registry checks were requested and notice from the administration at VA Central Registry office states that there is a delay in processing for all programs due to a backlog. We will be sure to request backlog notifications in writing in the future. As of 03/12/2020, there is still a backlog for Virginia.

Standard #: 63.2(17)-1721.1-B-3
Description: Based on record review, the child day center did not obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him.
Evidence:
Staff #3 did not have documentation of Maryland and Kentucky background checks in the record.

Plan of Correction: Fingerprint clearances were received prior to hire for all employees. Central registry checks were requested and notice from the administration at MD and KY Central Registry office states that there is a delay in processing for all programs due to a backlog. We will be sure to request backlog notifications in writing in the future. All other background checks remain in compliance during inspection. KY has been received as of 03/12/2020; however, there is still a wait for MD.
