Description: Based on staff interviews and review of documentation, it was determined that the center's own policies and procedures for identifying where children are at all times were not followed.


Evidence:
1. The center's transportation policy is for the driver of the bus to count the children as they exit the bus and to then walk the bus to verify that it is empty. Staff #1 stated she "did a count of the children getting off the bus but must have miscounted." Staff #1 did not check the entire bus upon exiting the bus on 1/14/2020 and Child #1 (Age 7) was left alone on the bus for approximately 5-10 minutes.
2. The center's name to face accountability policy states that name to face procedures are completed every time a threshold is crossed. The center's policy states, "Name to face accountability is when you look at your rollcall sheet and confirm by actually putting your eyes on each individual child marked present on the rollcall sheet. This procedure does not take the place of counting the number of children as you cross each threshold..." Staff #1 did not complete name to face accountability when the children transitioned from the bus inside the school to the vestibule area.

Description: Based on staff interviews and a review of documentation, staff did not always ensure the care, protection and guidance of a child.

Evidence:
On 1/14/2020 Staff #1 transported 11 children from 2 local public schools to the center. Staff #1 was unaware that Child #1 (Age 7) had not disembarked from Bus 231 with the rest of the other 10 schoolagers after arriving at the center from school. Child #1's guardian arrived to pick the child up from the center and asked center staff where the child was. Staff #1, who had transported the children, asked Staff #2 where Child #1 was and both staff checked the bathroom. Staff #1 then realized the child was still in the bus and ran outside. Staff #1 found Child #1 slumped over sleeping in the back seat of the bus. Staff #1 stated the bus was still running with the doors open when Child #1 was left unattended on the bus for approximately 5-10 minutes.

Description: Based on staff interviews, a child under 10 years of age was not always within actual sight and sound supervision of staff.

Evidence:
Child #1 (Age 7) was left unattended for approximately 5-10 minutes on a center bus after being picked up from a local public school and transported to the center. Staff #1 picked 11 children up from a local public school and was unaware that Child #1 (Age 7) had not disembarked from Bus 231 with the rest of the other 10 schoolagers after arriving at the center from school. Description: Based on staff interviews, it was determined that staff did not verify that all children had been removed from the vehicle at the conclusion of a trip from a local public school back to the center.

Evidence:
On 1/14/2020, Staff #1 put the bus in park and let 10 children off at the front entrance at the center. Staff #1 left the doors open and the bus running as she walked the children inside the center. Staff #1 did not walk the length of the bus and check the seats to ensure that all 11 children had gotten off the bus. Staff #1 stated she walked down the aisle and stopped between the second and third row of seats. There are 4 rows of seats in the bus. Child #1 (Age 7) had fallen asleep in the back seat and remained in the running bus unattended for approximately 5-10 minutes.
