Technical Assistance:
1. Discussed diaper changing requirements. 2. Reviewed/trained vendor on new subsidy requirements. 3. Discussed State license application processes. 4. Forms provided: emergency preparedness plan, emergency contact, transportation/trip, and proof of identity.

Comments:
10:15 - 11:45am Conducted an announced reinstatement subsidy inspection. There were no children present, and reportedly no children currently enrolled. An adult + 2 children that live in the home were present. Adult #1 translated between the Provider + inspector. The home was found to be clean, safeguarded appropriately, and sufficiently supplied with toys and equipment for children. Vendor training requirements have been met, and both the vendor and Adult #1 are currently certified in First Aid/CPR. One administrative non-compliance found.

Description: Based on records reviewed, it was determined that the home does not have a written emergency preparedness plan. Evidence: The emergency plan form was blank and not completed.

Plan of Correction: The vendor will complete the plan prior to enrolling children.
