Comments:
An unannounced monitoring inspection was conducted today from 9:40am-1:15pm. There were 56 children directly supervised by 9 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 6 staff records, 10 children's records, 6 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Adequate staff on site are current in CPR/First Aid, Daily Health Observation, and Medication Administration Training. Children were observed playing in centers, playing with play dough, learning colors and painting, completing an activity learning about plants and herbs, playing outside on the playground, and eating lunch which consisted of tomato soup, cheese bread, lettuce, pears, and milk. Infant feeding and diapering procedures were also observed. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-130-A
Description: Based on record review, 1 of 10 children's records reviewed did not contain documentation that the child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence: Child #6 did not have a complete immunization record on file.

Plan of Correction: Called parents to update medical records through fax from the doctors office.

Standard #: 22VAC40-185-70-A
Description: Based on record review, 1 of 6 staff records reviewed did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment.

Evidence: Staff #2 (Date of Hire: 2/10/2020) did not have documentation that two or more references were checked before beginning employment.

Plan of Correction: Called for references and I have them updated as of 3/12/2020.

Standard #: 22VAC40-185-510-A
Description: Based on review of 6 medications, not all medications on site were accompanied by a parental authorization granting the center permission to administer medication to the children.

Evidence: 3 of 6 medications were on site without parental authorization granting the center permission to administer the medication to Child #12, Child #13, and Child #14.

Plan of Correction: Will be getting parent signatures at pick up.

Standard #: 22VAC40-185-510-N
Description: Based on review of 6 medications and authorizations, it was determined not all procedures in regards to expired medication authorizations forms were followed.

Evidence: 1 of 6 medications reviewed have an expired medication authorization form with the medication still on site and was not returned to the parent within 14 days of the authorization expireation. Child #11's parental medication authorization form had effective dates of 1/28/2020-2/7/2020.

Plan of Correction: Will be returning the medication to the parent tonight and getting an updated medication authorization form.
