Comments:
A Subsidy Health and Safety inspection was conducted on today from 9:40am through 1:15pm to review supplemental health and safety requirements. A total of 56 children were present with 9 caregivers and all classes were in ratio. The children were engaged in the following: playing in centers, playing with play dough, learning about colors, painting, completing an activity with plants and herbs, playing outside on the playground, and eating lunch which consisted of tomato soup, cheese bread, lettuce, pears, and milk. Infant feeding and diapering procudures were also observed. Programming, menus, emergency procedures, 6 medications, and 10 children's records and 6 staff records were reviewed. Adequate staff on site are trained in CPR/First Aid, Daily Health Observation, and Medication Administration Training (MAT).

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-540-A
Description: Based on record review, 2 of 6 staff records reviewed did not have documentation of a negative tuberculosis screening or test submitted at the time of employment or that the test or screening submitted was completed within the last 30 days.

Evidence: Staff #2 (Date of Hire: 2/10/2020)'s tuberculosis test on file was dated 8/2/19 and Staff #5 (Date of Hire: 11/5/19) did not have tuberculosis screening or test on file.

Plan of Correction: I will be procuring the documents as of 3/12/2020 and sending Staff #2 for a TB test on 3/13/2020
