Comments:
A complaint was received involving care, protection and guidance ( dropping a child off at the wrong school) The investigation was done in conjunction with the local department of social services. The complaint was determined to be valid and additional violations were issued in the area of transportation. Please return a signed copy along with providing a plan of correction for each violation by 3/18/20.
Arrived at 2:20 pm and departed at 3:40 pm on 2/13/20

Description: Based on interviews, it was determined that the center failed to ensure the care, protection, and guidance of a 5 year old child.

Evidence:
On 2/10/20 at approximately 8:40 am staff A allowed child A, a five year old developmentally delayed child to get off the center's school van at school #1 along with 7 other children.

This was child A's first day riding the center's van to school and when staff A asked child A if this was his school, he said "yes"..

After dropping the children off at school # 1, the van drove off en route to drop off the remainder of the children at school #2. Two school personnel were out front of school #1 and immediately saw child A and realized that he was not enrolled at their school, however they were unaware of who had dropped the child off and the child was unable to provide any information about who had dropped him off at the school.

At approximately 8:50 am, the school personnel discovered that child A was dropped off by Discovery Learning Center and the school proceeded to call and notify the center of the error.

The center returned to the school at approximately 9 am to retrieve the child and dropped the child off at the correct school at approximately 9:10 am

Plan of Correction: 1. Transportation logs will be reviewed daily to manger to make sure all information is correct, prior to giving the logs to the teachers.
2. If any changes or additions were made to the log, the manager will notify the teacher of the changes or additions.
3. If there is as student that is new to our program, manager will introduce the student to the staff and will assign a classmate to be a buddy to help the new student. The buddy system will be in place for 2 weeks.
4. We have implemented a pre-boarding plan. The students will get in line by school and the staff will use name-face attendance prior to leaving the building and upon arriving at the school and getting off the bus.
The reverse will happen in the afternoon.
All staff have been retrained on transportation procedures.

Description: Based on interviews, it was determined that while providing transportation, the center was not responsible from the time a child boarded the vehicle until returning the child to the parent(s).
Evidence:
The center did not maintain accountability of child A when they dropped him off at the wrong school on 2/10/20.

Description: Based on interviews, it was determined that the center did not maintain a list of all of the children being transported in the center's vehicle.
Evidence:
While transporting child A to school on 2/10/20, child A's name was not on the Center's transportation list.
