Violations:
Standard #: 22VAC40-185-340-A
Complaint related: Yes
Description: Based on staff interviews, the facility failed to ensure that when staff are supervising children, they always ensure the care, protection, and guidance of the children. Evidence: on 1/16/2020, Staff A observed Child A (15 months) sustain an injury to the head after hitting her head on a piece of play equipment while on the playground between approximately 3:15 p.m. to 3:30 p.m. Staff A stated she "forgot to write an incident report" and "forgot to report the injury to management" before leaving at approximately 4:00 p.m. When the parent arrived at 5:30 p.m. to pick up Child A, none of the staff present knew how the injury happened. Staff B and Staff C, who provided care for Child A from 4:00 p.m. to 5:30 p.m., stated they did not see the injury until the parent picked up at 5:30 p.m. Staff B and Staff C did not know to monitor the child for any signs or symptoms related to the head injury because they had not been informed of the injury. The child was later seen by a medical professional and was diagnosed with a contusion.
