Comments:
An unannounced complaint investigation was initiated on February 3, 2020 and continued on February 5, 2020 with a representative of local agency in response to allegations made regarding supervision of children and incident reporting. Based on staff interviews and review of documentation, the preponderance of evidence gathered during the investigation supports the allegations. The complaint is determined to be valid. Description: Based on staff interviews, the facility failed to ensure that when staff are supervising children, they always ensure the care, protection, and guidance of the children. Evidence: on 1/16/2020, Staff A observed Child A (15 months) sustain an injury to the head after hitting her head on a piece of play equipment while on the playground between approximately 3:15 p.m. to 3:30 p.m. Staff A stated she "forgot to write an incident report" and "forgot to report the injury to management" before leaving at approximately 4:00 p.m. When the parent arrived at 5:30 p.m. to pick up Child A, none of the staff present knew how the injury happened. Staff B and Staff C, who provided care for Child A from 4:00 p.m. to 5:30 p.m., stated they did not see the injury until the parent picked up at 5:30 p.m. Staff B and Staff C did not know to monitor the child for any signs or symptoms related to the head injury because they had not been informed of the injury. The child was later seen by a medical professional and was diagnosed with a contusion.

Description: Based on interviews and review of documentation, the facility failed to notify parents immediately of a serious injury sustained while in care. Evidence: On 1/16/2020, Child A (15 months) sustained a head injury on the playground at approximately 3:15 p.m. to 3:30 p.m. The parents were not notified of the injury until the parent arrived to pick up the child at approximately 5:30 p.m. The parent saw the injury and asked Staff B and Staff C how it happened; Staff B and Staff C reportedly did not know how it occurred.

Evidence: on 1/16/2020, Staff A observed Child A (15 months old) sustain an injury to the head at approximately 3:15 p.m. to 3:30 p.m. Staff A did not write an incident report or report the injury to management before leaving at 4:00 p.m. When the parent arrived at 5:30 p.m. to pick up Child A, no one knew how the injury happened. An injury report was not documented until 1/17/2020.
