Comments:
An unannounced renewal inspection was completed on 3/17/2020. There were 134 children present with approximately 20 staff directly supervising and four administrative staff. Ten children's records and ten staff records were reviewed. Five medications were reviewed.

The inspector arrived at the center at 8:55 am and departed at 2:30 pm.

The children were observed during outside time, during classroom times, during lunch, and during nap. Infant activities were observed.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to ensure that all children's records contained documentation of all required information.

Evidence:
1. The record for child #1 had the biological father of the child listed as not authorized to pick up the child. There was no legal paperwork to support this.

2. There was no work information listed for the mother or father of child #2. The spaces for this information was left blank for both the mother and the father.

3. The address for one of two emergency contacts for child #5 was not complete. There was a road name and no house number for the address of the emergency contact.

4. There was no work phone number documented for the mother of child #6. There was a work place documented for the mother.

5. There were no work phone numbers listed for the mother and father of child #9. There were work places documented for both the mother and the father.

Plan of Correction: All was corrected during the inspection. The director had all information completed and removed the father's name based on information from the other custodial parent.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence:
1. There were two picnic tables observed on the school-age play area outside that were in disrepair. The tables and the bench seats were cracked and pieces of the hard plastic were broken in multiple areas on both tables creating jagged, sharp edges and pinch hazards. Both tables were observed being used during the inspection by school-age children.

Plan of Correction: The director will have the tables were removed during the inspection.
