Technical Assistance:
Consultation was given on CDC guidelines for the current health concerns, staff training for on-line DHO and scholarship opportunities.

Comments:
An unannounced monitoring visit was conducted today. All rooms were in ratio with direct care staff. Observed free play, infant care, transition to outside play time, physical plant, emergency preparation, staff and child records. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-240-A
Description: Based on record review, 1 of 3 staff have no documentation of receiving orientation by the end of the first day of assuming job responsibilities.
Evidence:
Staff #1 was hired 2/28/2020 and did not have orientation documentation

Plan of Correction: Orientation paperwork was completed by staff

Standard #: 22VAC40-185-240-D-5
Description: Based on staff administration statement, not all hours of operation are covered by a staff member with Daily Health Observation (DHO)
Evidence:
Only one staff has current DHO and the center is not staffed some afternoons.

Plan of Correction: Additional staff will be trained to cover all hours of operation.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas of the center, inside and outside are not maintained in a clean, safe or operable condition.
Evidence:
1. Stairwell has chipped paint
2. Rusted radiator in the upstairs bathroom
3. Frayed carpet on the stage in the grizzly class
4. Window frames peeling paint in the grizzly and polar bear classrooms
5. Exposed wire under the radiator in the brown bear class
6. Cracked black tube on the playground
7. Hanging wire off the shed on the playground
8. Plexiglass cracked on one pane in the grizzly classroom door.

Plan of Correction: Parent in-service day is scheduled for 4/25 to fix minor repairs/paint issues.

Description: Based on observation, cleaners were not kept in a locked place.
Evidence:
Cleaners were unlocked in the grizzly, sunbears and polar bears classrooms

Plan of Correction: Locking cabinets will be purchased for classes or use the cabinet in the room.
