Technical Assistance:
Consultation was given on the length of nap time is not to exceed two hours. Children should not be on cots for more than 2 hours.

Comments:
An unannounced renewal study was conducted today. All rooms were in ratio with direct care staff. Observed lunch, transition to hand washing and bathroom, moving to cots and the oldest group returning from a field trip. Reviewed a sample of records for children and staff, emergency preparation, training hours, nutrition, injury records and transportation requirements. Standard #: 22VAC40-185-60-A
Description: Based on record review, 2 of 3 children's records did not contain all required information.
Evidence:
Child #1 (enrolled 3/18/2018) and Child #3 (enrolled 3/9/2018) records did not have documentation of child updates and confirmation of up-to-date information in the child's record.

Plan of Correction: placed in their record.The children's updated information will be in their record.

Standard #: 22VAC40-185-270-A
Description: Based on observation, an area of the center was not maintained in a clean, safe or operable condition.
Evidence:
The wood fence in the two's area is splintered along one plank.

Plan of Correction: The small splintered area at the bottom of the fence along the perimeter of the playground has been reported to maintenance. The area is scheduled tor immediate repair. No children were in harm's way.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the diapering surface was used for more than diapering or cleaning children.
Evidence:
The diapering table had 7-10 music CD's on the surface of the table.

Plan of Correction: Staff has been counseled abot placing non-diapering items on the diapering table. A CD case was also purchase for the class.

Standard #: 22VAC40-185-550-E
Description: Based on observation and staff statement, a simulated drill was not recorded on the emergency log.
Evidence:
The Center Director stated that a second shelter-in place drill had been conducted with the children but had not been recorded on the log.

Plan of Correction: The date for the second shelter -in place has been recorded in the log book.
