Comments:

A subsidy health and safety inspection was completed on 03/03/2020 from 9:45am to 1:35pm to review supplemental health and safety requirements. The children were observed learning through play in centers, reading, completing craft projects and playing outside. A formal observation was completed with the Preschool and Pre-K classrooms. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today. Five children?s records and five employee records were reviewed during this inspection. Medication is being administered and was reviewed. Five injury reports were reviewed. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 02/18/2020 Last shelter-in-place drill: 03/02/2020 Last fire inspection: 02/13/2019 Last health inspection: 11/26/2019 Last Lockdown drill: 10/24/2019 Today, the following child to staff ratios were observed: Preschool (2.5 years-3 years) - 6:1 Preschool (Threes and Fours) ? 7:1 Pre-K (Fours and Fives) - 8:1 School Age - 14:1 A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. One violation was found during the supplemental subsidy health and safety inspection and are identified on the violation notice. Description:	Based on record review, the vendor did not ensure one of five staff records contained documentation of the Annual Virginia Health and Safety Update Training by March 1, 2020. Evidence: The record of staff #5 (start date: 3/14/15) did not contain documentation of the Annual Virginia Health and Safety Update Training.
Plan of Correction:	Staff completed training that day. Director created training checklist form to use to prevent this in the future. Admin. will oversee form.
