An unannounced licensing inspection was completed today.
Sixty-seven children were in care with 13 classroom staff, two office staff and a cook.
The children were playing outdoors, playing in centers, reading, eating lunch and resting.
Previous violations, medication, six staff records and eight children's records were reviewed.

Standard #: 22VAC40-185-160-A Description: Based on the review of six staff files the facility failed to ensure that staff have a TB test or screening.
Evidence:
1. There are no TB screening or test results on file for staff #4.Plan of Correction: TB screening for #4 required no later than 3/19/20 [sic]
Standard #: 22VAC40-185-270-A Description: Based on a tour of the outdoor play area the facility failed to ensure that all areas are in safe condition.
Evidence:
1. On the wooden climber where the green slide attaches to the deck, there is a screw which is loose creating an entanglement point.Plan of Correction: Screw tightened on 3/16/20 [sic]
Standard #: 22VAC40-185-330-B Description: Based on a tour of the outdoor play area, the facility failed to ensure that there is six inches of resilient surface material under climbing equipment and equipment with moving parts.

1. There is 1-3 inches under climbing equipment and swings.Plan of Correction: Mulch was ordered in 3/9/20. Extra mulch was ordered on 3/16. [sic]
