Comments:
An unannounced renewal inspection was conducted on March 13, 2020 with the center owner and staff to determine compliance with licensing standards. There were fifteen preschool children present with two staff directly supervising. The children were having rest time, playing outdoors, and eating afternoon snack. Snack served today: fresh fruit and graham crackers. Five children's files were reviewed - please see Supplemental Summary for details. Three medications for two children were reviewed. *Bus procedures were observed as the school age children arrived at the conclusion of the inspection. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of five children's files, the facility failed to have documentation of a physical on file for each child within 30 days of enrollment. Evidence: Child 1 did not have documentation of a physical on file.

Plan of Correction: Center staff will request the physical from the parents.

Standard #: 22VAC40-185-60-A
Description: Based on review of five children's files, the facility failed to have all required information in the file for each child. Evidence:
A. Child 1, Child 2, Child 3, Child 4, and Child 5 did not have documentation of
each parent's work phone number and place of employment,
name and phone number of each child's physician,
or the child's allergies or intolerance to food.
B. Child 3 did not have the name, address and phone number for a second emergency contact. Child 4 and Child 5 did not have the name, address and phone number for two emergency contacts.
C. Child 1, Child 2, and Child 3 did not have documentation of having viewed the child's proof of birth and identity.

Plan of Correction: The licensing inspector sent the center the VDSS model form titled "Child's Record and Agreement" to use as a template. The information in the automated system ProCare did not capture all of the required information.

Standard #: 22VAC40-185-510-N
Description: Based on review of three emergency medications for two children, the facility failed to return the medication to the parent within 14 days of the medication authorization expiring. Evidence: the long term medication authorization for Child A's medication expired 2/12/2020. A new authorization was not obtained and the medication was not returned to the parent.

Plan of Correction: A new long term authorization form will be obtained from the parent.
