Comments:
On January 31, 2020 the department received a complaint regarding ratios, supervision, behavioral guidance, parent communication, daily activities, building maintenance, and infant feeding. The licensing inspector conducted an unannounced complaint inspection on March 2, 2020 at the facility from approximately 8:40 AM until 1:50 PM. Interviews with staff were completed, observations were made at the facility and documentation was reviewed. On January 31, 2020 the licensing inspector conducted a telephone interview and reviewed documentation. On February 4, 2020 the licensing inspector conducted an additional telephone interview. The preponderance of the evidence gathered during the investigation supports one or more of the allegations; therefore, the complaint is determined to be valid. Complaint related: No
Description: Based on observations, the center did not ensure all hazardous substances were kept in a locked place. Evidence: 1. The licensing inspector observed two cans of disinfectant spray and one aerosol air freshener spray on an upper shelf in the two-year old classroom. 2. The licensing inspector observed a bottle of spray disinfectant and an aerosol can of carpet cleaning solution in an unlocked lower cabinet in the hallway outside of the infant and toddler classroom.

Plan of Correction: Administration will correct today. Staff will receive additional training as well.

Complaint related: Yes
Description: Based on a review of documentation and interview, the facility did not ensure that when supervising children, staff shall always ensure their care, protection, and guidance. Evidence: 1. The licensing inspector reviewed the record for Child #2 which indicated a current age of 10 months old. 2. During an interview with the licensing inspector on March 2, 2020, staff reported Child #2 fell from a highchair resulting in no injuries. 3. A written statement from Staff #8 states that on 11/18/19, Child #2 (who was 6 months old at the time) fell forward from a highchair when Staff #8 briefly turned their back to the child. Staff #8 indicated there were no injuries observed. 4. The licensing inspector reviewed documentation for disciplinary action resulting in the termination of employment for Staff #8. The Notice of Discipline reports the incident occurred on 11/18/19 and that Staff #8 was recommended for termination on 11/19/19. The licensing inspector observed the Notice of Discipline was signed by Staff #8 and administration.

Plan of Correction: Staff was terminated immediately.

Description: Based on observations and interviews, the center did not ensure staff to children ratio requirements applicable to the youngest child in the group were followed when children were regularly in mixed age groups. Evidence: 1. From approximately 8:40 AM to 9:00 AM the licensing inspector observed six infants and toddlers being supervised by Staff #6 in the dining area. 2. During staff interviews, it was reported that Staff #6 is scheduled to arrive at 7:30 AM and Staff #7 is scheduled to arrive at 9:00 AM for the infant and toddler classroom. Administration and staff reported there were both infants and toddlers who are regularly in care in the infant/toddler classroom. When children are regularly in ongoing mixed aged groups, the staff-to-children ratio applicable to the youngest child in the group shall apply to the entire group. The ratio should have been 1:4.

Plan of Correction: Administration will speak to staff about arriving at their scheduled times. There are new staff beginning this week as well.

Complaint related: Yes
Description: Based on observations, it was determined the center did not ensure that whenever two year old children were in care that there was one staff member for every eight children present. Evidence: 1. From approximately 8:40 AM to 8:50 AM the licensing inspector observed twelve two-year old children being supervised by Staff #5 in the dining area.

Plan of Correction: Administration will speak to staff about arriving at their scheduled times. There are new staff beginning this week as well.

Complaint related: Yes
Description: Based on observations, it was determined the center did not ensure that whenever children from three years to the age of eligibility to attend public school, five years by September 30; that there was one staff member for every ten children present. Evidence: 1. From approximately 8:40 AM to 9:00 AM the licensing inspector observed thirteen three and four-year old children being supervised by Staff #2 in the classroom and bathroom area.

Plan of Correction: Administration will speak to staff about arriving at their scheduled times. There are new staff beginning this week as well.
