Comments:
The licensing inspector conducted an unannounced renewal inspection on Monday, March 9, 2020 from 3:15pm to approximately 5:00pm. The program's site director and program director were present and assisted the inspector throughout the inspection. The census for today's inspection consisted of 106 children in the direct care of 10 staff members. During the inspection, the children were observed engaged in a variety of activities. The inspector completed formal observations throughout and interacted with the children when appropriate. All staff members in each grouping of children were observed having positive interactions with the children. All areas used by the program including classrooms used for the program, cafeteria, gym, hallways, and bathrooms were inspected. The program was equipped with supplies and items were available to the children. The menu was posted and today's dinner consisted of hot dogs, bun, veggie dippers, fruit, ranch, ketchup, and milk. Transportation is provided and one van was inspected. The first aid kit in the building and non-emergency supplies were inspected and found to be in compliance. Medication is administered and four medications and authorizations were reviewed today. During the inspection, ten children's records and staff information required to be present at the center for staff members were reviewed. The most recent evacuation drill was documented on 03/02/2020. The most recent shelter-in-place drill was documented on 03/02/2020.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of ten children's records and interview on 03/09/2020, the center did not ensure three children's records were complete.

Evidence: 1) The record for Child #1, enrolled on 02/03/20, did not contain the address for the second emergency contact listed. 2) The record for Child #9, enrolled on 09/03/19, did not contain the address for the second emergency contact listed. 3) The record for Child #10, enrolled on 09/03/19, did not contain the phone number or address for the second emergency contact listed. The record for each child enrolled shall contain the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.

Plan of Correction: The missing information will be obtained from each parent today.

Standard #: 22VAC40-185-510-D
Description: Based on a review of four medications and interviews on 03/09/2020, the center did not ensure that one medication authorization was available to staff during the entire time it was effective for one out of four children.

Evidence: 1) The licensing inspector observed Medication #1 at the center for Child #10. The parent requested Medication #1 be administered, but no authorization was present for this medication. 2) During interview, a member of management confirmed the required authorization was not present.

Plan of Correction: The center will request the authorization form from the parent today.

Standard #: 22VAC40-185-540-A
Description: Based on observation and interview on 03/09/2020, the center did not ensure to have a first aid kit wherever children are in care.

Evidence: 1) During the inspection of the van that was used for transportation on 03/09/20, the licensing inspector did not observe a first aid kit. 2) During interview, a member of management confirmed there was not a first aid kit available in the van during transportation on 03/09/20.

Plan of Correction: The program will purchase a new first aid kit for the van to ensure they are available on the vans used for transportation.
