Comments:
An announced initial inspection was conducted on March 16, 2020 with the licensee present. Four classrooms, two bathrooms and an outside area will be used by children. At this time, medication administration, transportation and food services will not be offered. Inspection will be reviewed by the licensing administrator regarding license issuance.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, facility did not ensure areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence: The following was observed during the tour of the facility and outside play area:
1. In the toddler room by the children's sink, the wood trim on the corner of a partial wall ended approximately 3' from the floor leaving a sharp wooden edge.
2. In the toddler room on the diapering counter, the edge stripping cover was missing leaving unfinished rough wood exposed.
3. In the toddler room along the bottom of the front wall, a portion of the floor rubber molding was missing leaving an exposed sharp plaster edging approximately 3" from the floor.
4. In the 2's room, the diaper changing pad extended 4" off the end of the counter causing the pad not to be completely secured on a surface.
5. There were two full size doors and a piece of plywood propped against the building immediately outside the door used for children to access the outside play area.

Plan of Correction: 1. Rubber placed on the edges of the wood trim.
2. Rubber was placed on the edge of the diaper counter.
3. Rubber was placed on the bottom of the wall.
4. New changing table was replaced with a portable one.
5. Doors were removed.

Standard #: 22VAC40-185-280-B
Description: Based on observation, facility did not ensure hazardous substances are kept in a locked place.

Evidence: In the children's bathroom located by the rear exit, hand sanitizers labeled "Caution and keep out of reach of children" were observed on the sink counter and in an unlocked cabinet under the sink.

Plan of Correction: Hazardous substances will be kept in a locked cabinet away from the reach of children. Was removed and placed in a locked cabinet during inspection.

Standard #: 22VAC40-185-310-F
Description: Based on observation, facility did not ensure there is a playground area for infants and toddlers that has at least 25 square feet of unpaved surface per infant/toddler on the outdoor area at any one time.

Evidence: The surface of the outside play area is a paved surface.

Plan of Correction: Take kids outside on a walk and buggy ride.

Standard #: 22VAC40-185-500-B
Description: Based on observation, facility did not ensure the diapering area had all required items.

Evidence:
1. The diaper disposal system required staff to push a button in order to open the lid. Diapers are required to be disposed of in a system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.
2. There was not a leakproof covered receptacle for soiled linens.

Plan of Correction: 1. The diaper disposal was replaced with a push bottom that opens the lid.
2. A leak-proof covered receptacle was purchased.

Standard #: 22VAC40-185-550-B
Description: Based on review and interview, facility did not ensure the written emergency preparedness plan did not address all required components.

Evidence: The emergency preparedness plan did not address emergency officer and backup emergency officer along with a 24-hour contact telephone number for each as well as staff training requirement.

Plan of Correction: Emergency plan was updated during inspection and staff training, emergency and emergency backup information was added.
