Technical Assistance:
Transportation standards and the definition of balanced mixed age groupings reviewed with the program director.

Comments:
An unannounced renewal inspection was conducted on 03/12/2020 from 10:15 am - 1:10 pm. At the time of the tour 68 children, ages two - six years, were observed in care with nine teaching staff and two administrative staff. Children were observed during morning program time. Records were reviewed for three staff and two board members. Eight children's records were reviewed in addition to all medication.
Violations were observed in four parts of the CDC standards and are listed on the violation notice.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on record review, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence:
1. The most recent immunizations on file for child 1, age two years, were last administered on 05/24/2018.

Plan of Correction: 1. Contacted parents & requested updated immunization records.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a safe condition.

Evidence:
1. One support pole for the circular saucer swing, on the preschool playground, lifts up when pushed or pulled on.
2. The landscape sheeting on the preschool playground is exposed in one area adjacent the metal climbing bars and the sand box.
a. This is a trip hazard.

Plan of Correction: 1. Will securely anchor poles to the ground to ensure stability and no movement. (04/01/2020)

2. Additional mulch laid to the playground to cover areas of concern & landscape sheeting. ( 03/20/2020)

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substances shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. A bottle of liquid correction fluid was observed stored on a cabinet, accessible to children, in children's house II.
a. Multiple warnings on the label indicate the product as a hazardous chemical.

Plan of Correction: Bottle of liquid correction fluid removed from shelf & placed in locked cabinet to prevent accessibility to students.

Standard #: 22VAC40-185-330-B
Description: Based on a review of the facility playground the center failed to ensure that when playground equipment is provided resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 as shown in Figures 2 (Compressed Loose Fill Synthetic Materials Depth Chart) and 3 (Use Zones for Equipment) on pages 6-7 of the National Program for Playground Safety's "Selecting Playground Surface Materials: Selecting the Best Surface Material for Your Playground," February 2004 and the fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall when the equipment is in use.
*The use zone requirement for stationary equipment is six feet on all sides of the equipment.

Evidence:
1. The metal climbing wall structure, on the preschool playground, has two pieces of equipment placed within the fall zone for this structure.
a. A wood see saw has been placed in the six foot fall zone to the rear of the climbing panel.
b. A wood table has been placed on the right side of the fall zone for this panel.
2. There is no resilient surfacing under and surrounding the metal dome climber on the back preschool playground.
3. There is no resilient surfacing at the bottom of the slide on the back preschool playground.

Plan of Correction: 1. Will move wooden walking structure & table to new location that allows for a fall zone around metal structure.

2. & 3 : Additional mulch was laid to the areas of concern mentioned.

Standard #: 22VAC40-185-350-E-7
Description: Based on observation, interview, and record review, the center failed to ensure that the ratio for balanced mixed-age groupings of children shall be one staff member for every 14 children.
"Balanced mixed-age grouping" means a program that is planned for children who enter the program at three through five years of age. The enrollment in the balanced mixed-age grouping comprises a relatively even allocation of children in each of three ages(three to six years) and is designed for children and staff to remain together with turnover planned only for the replacement of exiting students with children of ages that maintain the class balance.

Evidence:
1. There were a total of 21 children with two staff in children's house III.
a. One child in the group, child 1, is two years of age.
2. The classroom lacked a balanced mixed age grouping as the allocation of children included the following:
a. 1 two year old child;
b. 10 four-year old children;
c. 5 five-year old children;
d. 5 six - year old children.

Plan of Correction: 1 + 2: Additional staff present in classroom to maintain ratios.

Standard #: 22VAC40-185-380-A
Description: Based on observation, the center failed to ensure that there shall be a posted daily schedule that allows for flexibility as children's needs require.

Evidence:
1. There was no posted daily schedule in children's house II.
a. Staff in this classroom confirmed that the daily schedule was not posted.

Plan of Correction: Daily schedule posted & available for viewing.
