Description: Based on staff interviews and review of the employee orientation booklet, the licensee failed to maintain compliance with the center's own policies and procedures.
Evidence:
1. The Center's policy "Daily Routines-4 Diaper Changing" requires that "diapers shall be changed only in diaper changing areas?.
2. According to Staff #1, children that are toilet training are changed in the bathroom which does not include a diaper changing area.

Plan of Correction: All staff will be retrained on proper diaper and toilet training procedures. Staff are to follow correct center procedures for diaper changing, including only changing diapers in diaper changing areas. Children who are toilet training with soiled diapers/pull ups will be changed on changing table.

Description: Based on staff interviews and observation of the bathroom on the first floor, a nonabsorbent surface for diapering or changing was not used when diapering.
1. According to Staff #1, children that are toilet training and are still in diapers/pull-ups are changed in the bathroom. The bathroom was not equipped with a nonabsorbent surface for diapering or changing.

Plan of Correction: All soiled diapers/pull ups will be changed in accordance with the center's policy for "Daily Routines for Diaper Changing" which includes always using the proper changing area with nonabsorbent surfaces.
