Comments:
The licensing inspector conducted an unannounced complaint inspection in response to a complaint that was received by the licensing department on February 18, 2020 regarding the center's administration, nutrition, programming, supervision. The inspection was initiated on March 4, 2020 and completed on March 4, 2020. On March 4, 2020, the inspector was on site from 2:33pm to 4:45pm. Interviews with staff were completed and documentation was reviewed.

The preponderance of evidence gathered during the investigation supports part of the allegations; therefore, the complaint related to programming is determined to be valid, but the complaint related to administration, nutrition and supervision is determined to be not valid. Description: Based on record review, the center did not ensure to post a daily record for each infant, which can be easily accessed by both the parent and the staff working with the child that contained all of the required information.
Evidence:
1. The record of child #2, (start date: 10/28/2019), did not contain daily documentation of the infant's time spent on their stomach while the child was at the stage that they could not roll over.

Plan of Correction: The teachers have been instructed on how to put the tummy information in the app so that parents will be informed in real time. Tummy time is required on a daily basis.
