Comments:
The Licensing Inspector conducted an investigation in response to a self reported incident that was received by the licensing office on 3/05/2020 of a staff member rough handling an infant on 2/11/2020. Interviews were conducted with staff, a video recording was reviewed and staff and child records were reviewed. It was determined that a staff member roughly handled an infant in care. The information gathered during this investigation supports the allegations; violations were discussed with the center director. Standard #: 22VAC40-185-410-1
Description: Based on interviews, review of written statements and observation of video recording, a staff member used a forbidden action of rough handling an infant in care.
Evidence:
1. On 2/11/2020 in the afternoon, Child #1 (9 months old) was sitting in the crib. Staff #1 reached in the crib and took away something Child #1 had in their hand. Staff #1 then used her right hand to grab Child #1's left arm, picked Child #1 up off the crib by their left arm and flipped the child over head down. Child #1 began to cry.

2. Staff #1 stated she ?grabbed Child #1, one of the kids by his arm with one hand and flipped him, so he would lay down.? She stated, ?he was sitting down and kinda bops up and down and I didn?t want him to do that because it was time for him to go to sleep.? Staff #1 stated she watched the video previously and stated ?I feel like I was rough.?

3. Staff #2 stated that Staff #1 picked Child #1 up with one hand and pushed Child #1's head hard into the crib. Staff #2 stated that Staff #1 did the same thing to Child #2 and Child #3 on the same day. Staff #2 stated Staff #1 grabbed the child?s hand, pushed their heads down, to get them to go to sleep. Staff #2 stated she thinks the baby could have gotten hurt the way Staff #1 was handling the baby.

4. On 3/11/2020, Staff #2 emailed the inspector a statement about the incident observed on 2/11/2020 in which she wrote that ?I told her you can put him down he doesn't want to sleep. She ignore me and she keep doing what she doing." Staff #2 also wrote, "She grab his hand and put him down hardly and push his head down and she said {Child #1) you have to sleep.?; and ?cause he?s small I am sure it?t hurt.?

Plan of Correction: Staff #1 was terminated as a result of her actions. Staff #2 was terminated for failure to report as mandated reporter within a timely manner.
