Comments:
Licensing Inspector arrived at 11:15 AM and departed at 1:00 PM to conduct a monitoring inspection. At the time of the inspection, there were 10 children (ages infant through preschool) in care with three staff. Children were observed doing worksheets and participating in free play. The sample size was five staff and five children's records. Violations were reviewed with the Director at an exit meeting.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on review of five children's records, it was determined that the facility did not ensure that they obtained updated immunizations for all children under the age of two.

Evidence:
The most recent immunizations for child #2 (date of birth 8/16/18) was dated 7/12/19. Updated immunizations should have been obtained in January of 2020.

Plan of Correction: Center management will ensure that updated immunizations are obtained in a timely basis.

Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records, it was determined that the facility did not ensure that all children had a physical examination within 30 days of enrollment.

Evidence:
The record for child #1, date of enrollment 1/9/19 did not include documentation of a completed physical.

Plan of Correction: Center management will ensure that all children have a physical on file within 30 days of enrollment.

Standard #: 22VAC40-185-60-A
Description: Based on a review of five children's records, it was determined that the facility did not ensure that all required elements were present in each child's record.

Evidence:

The record for child #3, who was in care at the time of the inspection, did not contain addresses for the two emergency contact persons.

Plan of Correction: Center management will ensure that all children's records are complete.

Standard #: 22VAC40-185-570-E
Description: Based on observation, it was determined that the facility did not ensure that all prepared infant formula was dated and labeled with the owner's name.

Evidence:
There were prepared bottles of infant formula in the refrigerator that were not labeled and dated.

Plan of Correction: Staff will ensure that bottles brought from home are labeled and dated.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of five staff records, it was determined that the facility did not ensure that all staff had a completed search of the central registry (CPS check) on file within 30 days of employment,

Evidence:
The record for staff #5, date of employment 12/16/19, did not contain a completed CPS check.

Plan of Correction: Center management will ensure that CPS check is obtained and results placed in record.
