Comments:
An unannounced complaint investigation was conducted to investigate a complaint related to the supervision of children at the center. The inspection was initiated with the director and local agency staff on January 10, 2020 beginning at 2:45 PM and ending at 6:38 PM. A child interview, parent interview and four staff interviews a were conducted. Three staff records, one child's record, documentation, and video recordings were reviewed. An exit meeting was conducted on February 26, 2020. It was determined that the allegation was valid and a corrective plan was being developed by the center prior to the investigation. The owner and director spoke with the licensing inspector and member of local agency regarding the alternative security measures that would be implemented in order to ensure the safety of the children at the center.

Description: Based on a review of the facility's video recording and staff interviews, the center failed to ensure that all children under the age of 10 years were within actual sight and sound supervision of the staff. Evidence: On January 10, 2020 at approximately 11:50 AM a four year old child left the classroom being supervised by staff A and staff B. The child entered the kitchen area for approximately 3 minutes. The child left the kitchen area and exited the daycare center via the front door. The child was returned to the center by a staff member and was out of sight and sound supervision for approximately 6 minutes.

Plan of Correction: Bright Starts Daycare & Learning Center number one priority is the safety and supervision of our children. Bright Starts Daycare & Learning Center is committed to providing staff with every tool necessary to ensure the safety and supervision of our children. All staff will receive continuous training on safety and supervision of children including name to face count and active supervision (scan, move and interact techniques). In addition to our exterior door security system already in place Bright Starts recently implemented an interior door verbal audible chime system. The verbal audible chime system will alert staff when any door in the room is opened by audibly announcing the name of the door in the room that is opened. Bright Starts Daycare & Learning Center also installed child safety doorknob covers to certain interior doors. Bright Starts Daycare & Learning Center will also be extending the perimeter fence to ensure every exit door except the main entrance door will exit into the fenced in playground area. In addition to the Center office that is located right inside the main door Bright Starts will be installing a reception desk just inside the main door that will be staffed during operating hours with the sole purpose to greet parents and children coming and going in and out of the Center. Child safety and supervision is serious: we mean it and we live it each and every day.

Description: Based on a review of video records from the facility's surveillance camera and staff interviews, a staff at the center utilized a forbidden action of roughly handling a child. Evidence: Staff C was observed on the video recording from January 10, 2020 at approximately 12:00 PM roughly pulling a child by the arm to redirect their movement. The force of the pull caused the children's upper body to jerk towards the direction they were being pulled.

Plan of Correction: As part of overall staff training program additional emphasis will be placed on the following areas: redirection behavior, how to handle difficult children, classroom management and teacher/child interactions. Staff will be assigned to a much more experienced teacher that will continue to mentor and advance caretaking skills. Staff will be provided constant feedback by management through periodic evaluations.
