The licensing inspector conducted an unannounced inspection on Monday, February 24, 2020 in response to a self-reported incident that the Department received on February 5, 2020 regarding staffing and the supervision of children. The inspector was on site at the center from 12:15pm to approximately 3:30pm. Interviews were completed, observations were made, and documentation was reviewed. Violation of standards related to the supervision of children were found during the inspection and are identified on the violation notice.
Please complete the `plan of correction? and `date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from the date of receipt. You should specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
The provider's responses for the 'plan of correction' were not received as of April 3, 2020 and will not appear on this Violation Notice.

Standard #: 22VAC40-185-340-C Description: Based on a review of documentation and interviews, the center did not ensure during the stated hours of operation, there always shall be on the premises and on field trips when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means of communication between the two of them.
Evidence: During staff interviews on 02/24/2020 the following was reported: 1) Staff #1 reported that on 02/04/2020, the second staff member that was scheduled to open the center was a "no show." Staff #1 reported there were five children dropped off between the times the center opened at 6:00am until a second staff member, Staff #2, arrived at approximately 7:20am. 2) Administrator #1 reported that Staff #2 was alerted to assist Staff #1 and estimates that Staff #2 arrived at the center between 7:15am-7:20am.
3) A review of the center's sign-in sheet dated 02/04/2020 showed that Child #1 was dropped off at 6:10am, Child #2 was dropped off at 6:20am, Child #3 and Child #4 were dropped off at 6:55am, and Child #5 was dropped off at 7:15am.Plan of Correction: Not available online.
