Technical Assistance:
Technical assistance was offered regarding maintaining accurate medication administration documentation.

Comments:
An unannounced inspection was conducted in response to a complaint received on 3/10/20. The allegations were related to the the administration of medication for a child in care at the center. 4 staff members were interviewed, medication administration logs were reviewed, and medications on site were checked. Based on the preponderance of evidence, the complaint is determined to be valid. Areas of non-compliance are listed in the violation notice. Standard #: 22VAC40-185-40-E
Complaint related: Yes
Description: Based on records review and staff interviews, the licensee did not ensure that the center's activities were maintained in compliance with the center's own policies and procedures that are required by licensing standards.
Evidence:
1. The center has medication for Child # 1 that needs to be administered at 4:00 pm each day the child is present. The medication is a controlled substance.
a. The center has a "Documentation" policy that states "Log it. Record the date, time, and dose on the Medication Log. For controlled substances, record the new pill count." The medication log has a log that states ?New pill count received? where the staff member administering the medication counts the number of pills in the container before the medication is administered.
b. A review of the supplemental log for medication authorization for Child # 1 showed that on 11/19/19 the medication was administered by Staff # 1 and the pill count received was documented as 23, indicating there were 23 pills in the container at the time of administration.
c. The log showed that on 11/20/19, Staff # 1 administered the medication at 4:05 pm but wrote the same pill count (23) as 11/19/20. Staff # 1 stated that she may not have done a pill count on that day.
d. On 1/30/20, Staff # 3 administered the medication and documented the pill count received as 25. The log showed that Staff # 1 administered the medication on 2/5/20 which was the next time it was administered, and originally documented the count as 25, which was the same number as 1/30/20 indicating that a pill count was not done.

Plan of Correction: The MAT trained staff were retrained on Kindercare's policies when it comes to medication administration documentation and record keeping. A member of the management team will administer or will be present during the medication administration process to ensure that the appropriate documentation protocol is being followed.

Description: Based on records review and staff interviews, the center did not keep a record of medication including information about the staff member administering, and record of administration.
Evidence:
1. Child # 1 was given her prescribed medication on 1/4/20 at 4:00 pm.
2. The staff that administered the medication on this date did not sign under the "Staff Signature" column on the medication log.
3. Attendance records showed that Child # 1 was present at the center on 11/25/19 and 11/27/19. There was no written documentation on the medication log for these dates, showing whether or not Child # 1 received her medication.
4. Staff # 1 and Staff # 2 stated that if they had administered the medication on these days, they would have recorded the information on the log. These two days were unaccounted for, resulting in a medication error.

Plan of Correction: The MAT trained staff were retrained on Kindercare's policies when it comes to medication administration documentation and record keeping. A member of the management team will administer or will be present during the medication administration process to ensure that the appropriate documentation protocol is being followed.
