Discussed with Director: First Aid and CPR certifications, phone reference requirements, injury prevention plan, email address for background searches.

Comments:
An unannounced complaint investigation was begun today. Based on multiple staff interviews, the preponderance of evidence supports the allegation. The complaint is valid for ratios not always being maintained in the Toddler room. Other violations cited were found during the course of the investigation; however, not part of the original complaint. A Subsidy Health and Safety Inspection (SHSI) was also conducted today.

*In the Toddler room, there is peeling paint on red wall near the home living center.
*The soft climbing mat in the Bridge's room has multiple cracks and tears, revealing the foam inside.

Description: Based on multiple staff interviews, it was determined that the ratio of 1 staff to 5 children (1:5) is not always maintained in the Toddler room.
Evidence:
It was determined that in the mornings, the Toddler classroom had 6 to 7 toddlers with 1 staff, from 8:30 AM - 9:00 or 9:30 AM.
In the afternoon, after 4:00 PM when classrooms combine there were 16 to 18 Toddlers with 2 staff.

Plan of Correction: A Floater will be hired and placed in classrooms during high traffic times
