Comments:
An unannounced monitoring inspection was conducted on Friday, September 27, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 10:00 AM and concluded the inspection at 1:20 PM. The census for today's inspection consisted of 100 children. During the inspection, children were counting apple seeds, tasting different types of apples, reading books, building with magnetic blocks, writing in journals, eating snack and playing on the playground. The classrooms are decorated with children?s art work and equipped with age and stage appropriate materials. Children's equipment, learning materials, indoor space, restrooms, first aid kit and the nonmedical emergency supplies were reviewed. Medication is administered and medication, authorization forms and allergy plans were reviewed. Ten children's records and Ten staff records, annual fire and health inspections, fire drill, and shelter in place drill logs, daily attendance, and all required postings were reviewed.

The director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and can be found in the violation notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the center did not ensure that children?s hands are washed with soap and running water or disposable wipes after eating snacks.

Evidence:
In classroom #1, #6, #7, #8, children finished eating snack and began the next activity without washing their hands with soap and running water or disposable wipes.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the facility did not ensure diapering standards were followed.

Evidence:
1. The mat on the changing table in classroom #2 had a tear in the vinyl covering approximately one and half inches long. The nonabsorbent foam padding was exposed.
2. The plastic-lined storage system had a broken operational foot operated pedal. The standard requires that the plastic lined storage system is either foot-operated or used in a way that neither the staff member's hand nor the soiled diaper touches the exterior surface of the storage system during disposal.
3. The plastic-lined storage system had multiple soiled diapers deposited in it.

Description: Based on observation, the center did not ensure that the surface of tables were sanitized using a spray disinfectant solution after each use for feeding.

Evidence:
