Comments:
An unannounced complaint inspection was conducted on 03/03/2020 in regards to information received regarding non-compliance with ratios. An inspection was conducted and there were 6 violations, in 3 separate areas, cited. This complaint is deemed valid. If you have any questions please contact me. Standard #: 22VAC40-185-70-A
Complaint related: No
Description: Based on observations and discussions with staff the facility failed to maintain compliance with this standard as required relating to staff having a staff record while working.

Evidence:
Staff #1 arrived from cooperate at approximately 8:09 am on 03/03/2020 to assist. Staff #1 did not have a file as required. Staff #1 was assisting with the children and with cooking during this inspection.

Plan of Correction: The HR Director will ensure that all corporate staff that would need to go into a center for an emergency that they have the appropriate paperwork before doing so. 4/1/2020

Description: Based on observations and discussions with staff the facility failed to maintain compliance with this standard as required relating to ratios when children are in mixed age groupings.

Evidence:
At approximately 7:44 am there were 7 children in care and the youngest was 17 months. By approximately 8:04 am there were 13 children in care with one staff member and the youngest child was 17 months. Based on the youngest child, the required ratio would be 1 staff to 5 children. Staff verified that this is a normal occurrence most mornings as they are short staffed.

Plan of Correction: The DOO has hired 2 staff and adjusted staff schedules to ensure that we stay in the required ratios. 3/13/2020

Description: Based on observations the facility failed to maintain compliance with this standard as required related to infant ratio.

Evidence:
From approximately 8:09 am-8:15 am on 03/03/2020 there were 5 infants in care with 1 staff.
The required ratio was 1 staff to 4 children.

Plan of Correction: The DOO has adjusted staff schedules to ensure that we stay in the required ratios. 3/13/2020

Description: Based o observations and discussions with staff the facility failed to maintain compliance with this standard as required relating to moving of children to different classes to aide in ratio compliance.

Evidence:
The LI observed numerous children being moved around to different classes between approximately 7:32 am-8:52 am on 03/03/2020 to try to aide in ratio issues during this time. Staff verified that this is a normal occurrence most mornings as they are short staffed, especially on Tuesday and Thursday until around noon. Most of the children were moved between the older 3 classrooms during this period of time.

Plan of Correction: The DOO has hired 2 staff and adjusted staff schedules to ensure that we stay in the required ratios. 3/13/2020

Description: Based on observations and discussions with staff the facility failed to maintain compliance with this standard as required related to background checks of staff.

Evidence:
Staff #1, from corporate, arrived at the facility at 8:09 am on 03/03/2020 to assist with care of the children and cooking. This staff did not have a sworn disclosure or a fingerprint letter as required.
This staff was still present assisting at the facility when the LI finished this inspection at 9:45 am.

Plan of Correction: The HR Director will ensure that all corporate staff that would need to go into a center for an emergency that they have the appropriate paperwork before doing so. 4/1/2020

Complaint related: No
Description: Based on observations and discussions with staff the facility failed to maintain compliance with this standard as required related to renewal of background checks of staff.

Evidence:
Staff #2, agent and acting center director, did not have a renewed Central Registry Clearance as required. The one on record had expired as of 02/10/2020. This staff had been and was currently working at this facility at the time of inspection.

Plan of Correction: We have received the Central Registry back and the HR Director will come up with a system to ensure that these stay up to date. 4/1/2020
