An unannounced complaint inspection was conducted on 2/28/2020 in response to a complaint received in our office regarding forbidden actions. Based on record review, staff interviews, and a review of the center?s policies, the complaint is unfounded. Other areas of non-compliance have been found are noted on the Violation Notice. Standard #: 22VAC40-191-60-B Complaint related: No Description: Based on a review of staff records, an employee was employed before having their completed sworn statement or affirmation.
Evidence:
1. Staff #4?s (doh: 12/10/2019) sworn statement on file was dated 1/5/2020.Plan of Correction: The sworn statement will be completed immediately after inspection and added to staff #4's file.
Standard #: 22VAC40-191-60-C-2 Complaint related: No Description: Based on a review of staff records, the center did not deny continued employment of an employee that did not have a central registry finding within 30 days of employment.
Evidence:
1. Staff #4 (doh: 12/10/2019) central registry findings on file were dated 1/28/2020.Plan of Correction: If we find in the future that the central registry background check is taking longer then 30 days for an employee: we will partner with the central registry office for their support and pause employment if needed.
Standard #: 63.2(17)-1720.1-B-2 Complaint related: No Description: Based on a review of staff records, staff did not obtain fingerprint results prior to the first day of employment.
Evidence:
1. Staff #4?s (doh: 12/10/2019) fingerprint results on file were dated 1/8/2020.Plan of Correction: We will ensure that all staff are sent for fingerprinting and the results received before we extend any employment offer.
