Technical Assistance:
Food service and USDA requirements were discussed. Current requirements state that each meal must include a protein, a grain, a vegetable, a fruit and milk. Lunch must have BOTH a fruit and a vegetable. If jelly sandwiches are served, there still must be a protein component served.

Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector was in the facility between 11:15 AM and 2:00 PM. Morning activities, lunch service and nap time were observed. A sample size of four staff records and five children's records were reviewed. It was reported that no medication is administered to children in care and no children's injuries were reported.

Violations:
Standard #: 22VAC40-185-40-E
Description: Based upon review of policies and staff interview, the facility has not ensured that the activities comply with their own transportation policies.
Evidence:
1. According to staff 1, children have recently been and are currently being picked up from public school using an Uber driver. Staff 1 stated that each day Uber is contacted to provide a driver with a large capacity vehicle in which the children will be transported from elementary school to the facility. Staff 1 stated that a current staff person rides with the Uber driver to pick up the children.
a. The facility's Transportation of Children policy states in regard to the Qualifications for Drivers that "in addition to meeting general staff background check standards, any driver who transports children for any purpose should have a valid driver's license and a safe driving record for more than 5 years, with no crashes where a citation was issued, as evidenced by the state department of Motor Vehicles record.
Staff 1 stated documentation of driver background checks, licenses and driving records are not obtained and that it is assumed that Uber has checked these credentials.
2. In addition, the facility's transportation policy as given to parents states that "The driver's license number and date of expiration, vehicle insurance information, and verification of current state vehicle inspection should be on file in the facility".
a. Staff 1 verified that documentation of driver's licenses, insurance and vehicle information is not on file in the facility for the Uber drivers/vehicles.

Plan of Correction: The facility responded with the following:
As discussed with inspector, I was not aware that I'm suppose to obtain records for each Uber/Lyft driver I use. occasionally.
Uber/Lyft.com shows all their drivers meet the stated requriement.
I have updated my transportation policy to reflect the use of Uber/Lyft occassionally and staff will verify DL registration and insurance information each time.

Standard #: 22VAC40-185-60-A
Description: Based upon review of five children's records and staff interview, the provider has not ensured that each child's record includes the name of any additional programs or schools that the child is concurrently attending and the grade or class levels.
Evidence:
1. In the record provided for child 5 , the parent indicated that child 5 attends Point O' View Elementary School. Staff 1, in describing the before and after school drop-off and pick-up procedures, stated that child 5 is currently attending Arrowhead Elementary School.
2. Staff 1 verified that the record for child 5 indicated Point O' View Elementary School and that there was no additional documentation of the school child 5 is currently attending.

Plan of Correction: The facility responded with the following:
Child is initially zone for Point O VIew and was supposed to attend Point O View at the time of enrollment. School changed afterwards and provider forgot to update child's file. The child's file has been updated to reflect Arrowhead Elementary.
