Comments:
An unannounced monitoring inspection was conducted Thursday, February 6, 2020 to determine the center's compliance with licensing standards. The inspector arrived at 10:46 AM and concluded the inspection at 12:40 PM. Today, there were 22 children in attendance. During the inspection, children were coloring, singing, eating lunch, taking bathroom breaks, washing hands, and napping. Children's equipment, learning materials, indoor and outdoor space, restrooms, first aid kit and nonmedical emergency supplies were reviewed. Medication is administered, and medication, authorization forms and allergy plans were reviewed. Five children's records and five staff records, fire drill and shelter in place drill logs, daily attendance and all required postings were reviewed.

A member of management was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and can be found in the violation notice.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on a review of staff records, the center did not ensure that subsequent communicable tuberculosis (TB) screenings were conducted at least every two years from the date of the initial screening.

Evidence:
The record for staff #4 (date of last TB screening 05/15/2017) did not contain updated TB screening results as of 02/06/2020.

Plan of Correction: Called and scheduled a T.B. health screening appointment today and check staff files yearly to keep them current up to date.

Standard #: 22VAC40-185-270-C
Description: Based on an observation of the classroom thermostat, the center did not ensure that inside areas occupied by children shall maintain a temperature no lower than 68 degrees fahrenheit.

Evidence:
In classroom #1, the wall mounted thermostat had a temperature reading of 64 degrees fahrenheit.

Plan of Correction: Check temp. in each classroom daily upon arrival.

Standard #: 22VAC40-185-570-A
Description: Based on observation, the center did not ensure that when a child is placed in an infant seat or high chair, the protective belt shall be fastened securely.

Evidence:
1 In classroom #2, two highchairs were in use, the children in the highchairs did not have the protective belt fastened.
2 The teacher stated that the protective belts were not fastened at the time of inspection.

Plan of Correction: Make sure all children sitting in high chairs are buckled in at all times for safety.
