A complaint investigation was initiated on March 13, 2020 regarding playground safety, behavioral guidance, and unclean physical plant conditions. Interviews were conducted, observations were made, and documentation was reviewed. There was not a preponderance of evidence to support the allegations regarding behavioral guidance and playground safety; however, the complaint was deemed valid in regards to unclean physical plant conditions.

Standard #: 22VAC40-185-270-A Complaint related: No Description: Based on observation, the center did not maintain the facility in a clean operable condition.
Evidence: Mold in found on the wash bin in the storage room and outside playground equipment.Plan of Correction: The playground equipment has been cleaned and sanitized there is no longer evidence of mold on the equipment.
The wash bin was cleaned with ECO Lab cleaning products as well bleach. The baseboards in the wash room have also been cleaned. Further corrective action includes installation of FRP which is washable wallboard surface that won't attract or absorb mold.
